The Program has contracted with chiropractors to provide services to students and dependents covered by the BYU Student Health Plan at a reduced fee schedule. Please understand this is not an insurance program, you will be responsible to pay the chiropractor for the services you receive at the time of your visit. However, by using the Program, you will pay 10% to 50% less than you would have paid otherwise. Using the Discounted chiropractic part of the Program is straight forward, simply:
• Schedule an appointment with one of the contracted providers listed below.
• Be sure to tell the office that you are a part of the BYU Student Health Plan Discounted Services Program when
you make the appointment.
• Bring your student ID and DMBA membership cards to the appointment.
•You must pay for the services you receive at the time of service. The exact amount you will owe depends on what services you receive. The price for most common services are listed below. Services not on the price list are 80% of the provider's usual charge. You may want to print out the price list and take it with you to the provider's office.You can also ask the office if its possible to estimate the amount you will owe before they start to provide services.
Because the Program is not insurance, there are no claim forms, enrollment procedures, benefit limitations and conditions, etc. You and the chiropractor determine what services you will receive, and you pay for those services at the time of your visit. Your visit may involve more than one service code. For example, a common set of service codes for a new patient at the first visit is the initial new patient exam ($25) and also an adjustment ($27), so the total amount owed the provider would be $52. If you also received a cervical radiology exam ($45), the total amount owed would be $97.
Also, if you were in treatment with a contracted provider prior to the time you had access to the Discounted Services Program, the discounted price is only applicable to the services received after you had access to the Discounted Services Program. And, coupons, specials and other types of offers promoted by contracted providers may be used in conjunction with this Program only at the discretion of the contracted provider.
