	Participants in the Dental Savings and Wellness Support Program (Program) acknowledge that there is a possibility that health care professionals and others providing services in connection with the Program can cause injury and damage to a Participant. The Participant understands and accepts such risk. Koster Insurance Agency, Inc. and its Program contractor, Basix, disclaims any warranty, expressed or implied, regarding the quality, nature or kind of services to be rendered to a Participant by any health care professional providing services under the Program. Koster Insurance Agency, Inc. and/or Basix shall not be responsible or liable to the Participant under any circumstances for any injury or damage sustained by the Participant as a result of services provided under the Program by any health care professional or other provider. Koster Insurance Agency, Inc. and/or Basix shall not be liable for any damages, including without limitation, direct, indirect, consequential, incidental, special or exemplary damages arising out of or related to the provision of services to the Participant by a health care professional or other provider under the Program. You agree that you will use Basix, LLC downloads and all other information contained on any of our websites only for your own personal, non-commercial use. You agree not to assign, copy, transfer or transmit all or any part of the Basix, LLC downloads or any other information contained on any of our websites to any third party. Further, you understand that using our downloads and/or any other information contained on any Basix, LLC web site, can cause injury, temporary or permanent disability or death and you accept those risks.
